Wylie ISD Athletic Physical Statement for the 2020-2021 school year

Athletes who participated in a school-sponsored UIL sport during the 2019-2020 school year, or
had a physical performed within the past calendar year, will be approved for the 2020-21 school
year upon the completion of a new medical history without any questionable
concerns/conditions (to be determined by a WISD Athletic Trainer).

Exceptions as follows:
- Any answer of “yes” to questions 1-6 of the medical history must be reviewed by
a Wylie ISD Athletic Trainer and requires medical clearance which may include a
new physical.
- All incoming 7th-grade athletes are required to obtain a new athletic physical
dated after the 1st of April, 2020.
- Any athlete who did not compete in a school-sponsored UIL sport during the
2019-2020 school year will require a new physical exam.

ALL Athletes are required to submit a new Medical History Form before any
participation and must use the most recent UIL Medical History Form dated 2020.

ALL athletes will ALSO need to complete all other required athletic paperwork,
available online only at http://wylieisd.rankonesport.com

Athletic Physical/Medical History Submission:
Completed current UIL medical history and, if required, the physical exam should be uploaded
into RankOne at http://wylieisd.rankonesport.com . The website will require two pictures to be
uploaded because it is expecting a medical history and physical. If you are only needing to
upload one document, please upload the same picture twice. Once a picture of the form is
uploaded it will be reviewed and approved or denied by a WISD Athletic Trainer.
